As the parent/guardian of the above player I agree to the following liability release: Participant and Guardian/Parent agree that New South Volleyball, its coaches, employees and agents shall not be held liable for any claims, demand, cause of action of any kind whatsoever, for or on account of death, personal injury, property damage or loss of any kind resulting from or relating to participants' use of facilities or participation in any sport, exercise or activity within or out of the New South Volleyball premises, and participant agrees to hold New South Volleyball harmless for the same. I give my permission to have the above named child to receive any emergency treatment deemed necessary while on the New South Volleyball Club premises should the need arise.

Day of Clinics

Please make sure you bring the $10 if you are a New South VBC players, or $20 for non- New South VBC players. (cash or check)
Athletes will need to wear comfortable athletic attire. Kneepads are encouraged but not required.
Please arrive at least 30 minutes early for check-in and warm ups.
Please bring all USAV paperwork…including Medical Release, membership form etc…
