OPEN GYM

I agree to the following release of liability: Participation specifically agrees that New South Volleyball Club, its employees, coaches and agents shall not be held liable for any claims, demand, cause of action of any kind whatsoever, for or on account of death, personal injury, property damage or loss of any kind resulting from or relating to participants' use of facilities or participation in any sport, exercise or activity within or out of the New South VBC premises, and participant agrees to hold New South VBC harmless for the same. I also give my permission to have the above child receive any emergency treatment deemed necessary. *

$10- 1 open gym session for nonmember

$20- 2 open gym sessions for nonmember

$30- 3 open gym sessions for nonmember

Dates Attending Open Gym (please identify which dates you will attend: *

New South VBC will be hosting 3 open gym sessions (11/8, 11/15 & 12/6 of 2015) at West Creek High School from 1-3 pm. These sessions are open to any and all volleyball players ages 12-18 years of age. Participants must register to attend open gym. These sessions will provide both skill drills as well as open play. Open gym sessions are free to players with the New South Volleyball Club and are $10 per session for nonmembers. Payment may be made on site via cash or check (made out to New South Volleyball Club). We look forward to continued volleyball opportunities for the Clarksville-Montgomery Co/Fort Campbell and surrounding areas!
